IMPORTANT INFORMATION - KEEP FOR FUTURE REFERENCE

The 2008 Annual Convocation and Dinner Dance of the U.S.A. Section of the International College of Dentists will be held this year on Friday, October 17, 2008 at the Grand Hyatt Hotel, 600 E. Market Street, San Antonio, Texas 78205. All of our meetings/events will be held there and it is within easy walking distance of all ADA functions. There are three ways to register for the ADA annual session and/or secure hotel reservations - online, mail and fax. Please find further hotel registration information below. If the ICD uses up its allocated room block, we benefit from a reduced cost for many items; therefore, it is to our advantage for you to use our housing code (online ICDSAT; or hard copy A-ICD) for your reservations.

The Convocation and Induction Ceremony will begin at 1:00 p.m. on Friday afternoon in The Texas Ballroom. No tickets are issued for this ceremony which is open to all Fellows, families and guests. Dr. Kenneth L. Kalkwarf, Dean, The University of Texas Health Science Center at San Antonio Dental School, is the scheduled speaker for the Convocation. The doors will open at 12:00 noon.

There will be considerable formality during the Friday evening Black Tie Optional Dinner Dance program. The Social Hour (cash bar) begins at 6:30 p.m. in the Texas Ballroom Pre-Function Area. Dinner will start at 7:30 p.m. in The Texas Ballroom followed by a stage show and dancing. Our band this year is extra special. Max Class and the Class Act (http://www.maxclassact.com) is a premier party/variety dance and show band. This high energy group will amaze you with their spectacular show, exciting stage presence, versatility and colorful costumes. They are phenomenal with a repertoire that includes top hits from the forties to the nineties. Plan to stay after dinner to enjoy this Las Vegas-style entertainment.

If you will be joining us at these ICD functions please return the above reservation card along with your payment of $100 per person for dinner, by check (payable to the ICD) or credit card. Mail the completed card and payment to International College of Dentists, 51 Monroe Street, Suite 1400, Rockville, MD 20850-2412 or fax it to 301-738-9143. No tickets are mailed. Tickets are held in the name of the purchaser and must be picked up at the ticket desk in the Texas Ballroom Pre-Function Area between 9:30 a.m. and 7:15 p.m. on Friday, October 17. Your cancelled check/credit card statement will serve as your confirmation. Refunds will only be given on cancellations made before October 3, 2008.

Fellows-Elects should NOT use the downloadable card above. They MUST use the blue card they will receive in the mail.

Tables seat ten (10) and will be assigned by District. If you wish to sit with friends, please confirm with them and write each name and state on the seating preference portion of the reservation card. Where no other request is made, seating is by State and District groups, when possible. Your table number will appear on your ticket. The Fellow-Elect is a guest of the College but must personally return a reservation card; their spouse/guest must purchase a dinner ticket. We will try to honor late requests but only those received before the September 12, 2008 deadline can be guaranteed.

HOTEL RESERVATION INFORMATION
There are three ways to secure Grand Hyatt Hotel reservations (map code 11) for our Annual Meeting in San Antonio, TX through Experient, not the ICD - online, mail and fax. Our block of rooms fills quickly - It will be held until July 3, 2008 or until rooms are exhausted. Please book your hotel reservation as soon as possible using our code ICDSAT for online reservations; or use our code A-ICD for hard copy reservations (see links below).
