Dear ICD Fellow:

On Friday, October 2, 2009, the U.S.A. Section of the International College of Dentists will hold its Annual Convocation at 1:00 pm and Dinner Dance at 6:30 pm in the Hilton Hawaiian Village Hotel, 2005 Kalia Road, Honolulu, HI 96815. You may reserve a room in our hotel block by following the instructions below. At a later date, by email and mail, you will receive a dinner dance reservation form and further details.

There are three ways to secure reservations at the Hilton Hawaiian Village Hotel in Honolulu, Hawaii through Experient (not the ICD) - online, mail and fax. Our block of rooms fills quickly - It will be held until June 18, 2009 or until rooms are exhausted. If the ICD uses up its allocated room block, we benefit from a reduced cost for many items; therefore, it is to our advantage for you to use our housing code / password (ICDHNL) for your reservations (see below).

To register and/or reserve rooms in our block, print these instructions. Either follow this link, http://registration.experient-inc.com/showden091/Default.aspx?link=AFFILIATE or copy and paste the link in your web browser. Type our Password, ICDHNL (all caps), in the box next to the red asterisk.

IMPORTANT: If you plan to attend ONLY ICD functions, and do not wish to register for ADA events, follow these directions to avoid a registration fee: After typing in our Password, ICDHNL, click on "Continue". Scroll down to "Register Now" and click on "Start Here". On the "Record Lookup" page, SKIP 1) Dentist Record Lookup (for people attending ADA activities), and SCROLL DOWN TO 2) Dental Professionals and Constituent and Component Association Staff. Click on, "Click Here to Register". Fill out your Profile including the three questions below your email address, then click on "Next". Under Registration Type, choose Housing Only - No Badge Issued ($0 Registration fee).

Should you wish to register and/or make hotel reservations utilizing the hard copy form, you may do so by writing our Code ICDHNL in the top, right corner of the form. Forms submitted without our code will not be granted rooms in our block. Go to http://www.ada.org/goto/session and click on "Complete Your Registration and Housing" toward the top of the page. Scroll down and click on "Group Housing Blocks". Click on Registration and Housing Form "PDF File 45/k". If not attending ADA functions, next to Advance Registration & Primary Registrant Codes and Fees, write in "Affiliate Housing Only - No Fee". Registration and Housing - Changes or Cancellations
Hotel cancellations and/or changes must be received in writing by Experient before 5:00 pm Central Time on Thursday, September 3, 2009. Cancellations received by this date will receive a full refund of the hotel deposit by Experient. Please allow 60 days for refund processing. Failure to check-in to your hotel on the confirmed date of arrival will result in the forfeiture of the entire room deposit. As of Thursday, September 10, any changes to hotel reservations must be made directly with the hotel. Hotel deposit refunds will not be issued after September 3.

Advance registration for the Annual Session closes at 5:00pm Central Time on Thursday, September 3, 2009. Registration, ticket and/or special event cancellations must be received in writing on or before September 3, 2009 for a full refund, less a $10 processing fee per cancellation. No refunds will be given for registration cancellations after September 3, 2009 for any reason. Please allow 60 days for refund processing.

ADA Registration
Starting March 31st, you can also access an electronic copy of the Preliminary Program for the 150th ADA Annual Session to be held Sept 30-Oct 4 in Honolulu, HI at http://www.ada.org/goto/session. To register for the Annual Session, please select the appropriate registration category and corresponding fee. You may also register friends and family. Add continuing education courses to your registration and sign up for special events beginning March 31, 2009. All courses, both fee and non-fee offerings will be ticketed. To attend any course, you must present your ticket. After September 3, 2009, all fees will increase and registration badge materials will not be mailed.
