IMPORTANT ANNUAL CONVOCATION, DINNER DANCE INFORMATION - KEEP FOR FUTURE REFERENCE

Dear Fellow:

The 2009 Annual Convocation and Dinner Dance of the U.S.A. Section of the International College of Dentists will be held this year on Friday, October 2, 2009. All ICD functions will take place on the premises of Hilton Hawaiian Village, 2005 Kalia Road, Honolulu, Hawaii 96815. We hope to see you this fall as we gather to honor our new Inductees. Make your plans to join us NOW.

Our Convocation and Induction Ceremony will begin at 1:00 pm on Friday, October 2 in Tapa 1 & 2 (Hilton Hawaiian Village - Tapa Tower.) Doors open at 12 noon. No tickets are issued for this impressive ceremony which is open to all Fellows, families and guests.

Friday evening, our black tie optional program honoring our new inductees begins with a Social Hour (cash bar) at 6:30 pm in the Coral Lounge (Hilton Hawaiian Village - Mid Pacific Conference Center). Dinner begins at 7:30 pm in Coral Ballroom 3-5. Plan to stay after dinner to enjoy an evening of dancing.

If you will be joining us, please mail the above Dinner Dance reservation card to ICD by August 28. No tickets are mailed - Your cancelled check/credit card statement will serve as your confirmation. All tickets are held in the name of the purchaser and must be picked up at the ticket desk on October 2 - either from 9:30 am to 4:00 pm at the Palace Lounge outside Tapa 1 & 2 (Tapa Tower) OR at the Coral Lounge (Mid-Pacific Conference Center) between 4:30 am and 7:30 pm. Your table number will appear on your ticket(s).

Dinner tables seat ten (10). If you wish to sit with friends, please write each name and state (if state differs from yours) on the reservation card. Otherwise, seating is by State and District groups, when possible. The Fellow-Elect is a guest of the College and must personally return a reservation card to assist with record keeping; their spouse/guest(s) dinner tickets must be purchased. We will try to honor late requests but only those received before the August 28, 2009 deadline can be guaranteed. Dinner dance cancellations received by September 14, 2009 will receive a full refund.

If you did not secure Hilton Hawaiian Village hotel reservations and/or register for the ADA Annual Session through Experient after receiving our previous annual meeting correspondence, you should do so as soon as possible.
